SERVICE DESCRIPTION:Links families of children with special health care needs to a network of quality providers and helps families obtain payment for the services their children need.
HOW SERVICE IS PROVIDED:Phone, onsite, home visits
ELIGIBILITY:Assistance is provided to individuals under the age of 26 who have special health care needs and are residents of the State of Ohio.
Determined case by case based on: a percentage of the federal poverty income guidelines, the family's taxable income, the medical care the child needs, a calculation of the family's maximum ability to pay for healthcare, amount spent on private health insurance, and amount spent on weekly childcare.
