SERVICE DESCRIPTION:Helps adults access dental care by matching eligible clients with volunteer dentists who provide either donated or discounted dental care in their private offices.
HOW SERVICE IS PROVIDED:Provided in private dental offices when matched to a dentist. Accessibility is dependent on the volunteer dentist’s office. Some offices may not be accessible for wheelchairs but if the client is eligible for the program, we would ensure the dentist with whom they’re matched can accommodate them.
ELIGIBILITY:Older adults, people with disabilities or serious medical conditions, as well as adults with lower incomes (< 200% of poverty) and severe dental needs. Clients must be uninsured for dental care, including Medicaid.
SERVICE HOURS:Clients may contact the program coordinator Monday through Friday, 7am-4pm. If matched to a dentist, client will be responsible to make an appointment with the dentist during their office hours which may vary.
FEES:If client is eligible for the discounted portion of the program, they will need to pay (minimal fees) for the services at the time they’re rendered. Otherwise, there is no cost for those eligible for donated services.
DOCUMENTS REQUIRED:Verification of income is required. If a client’s income indicates they would be eligible for Medicaid, the program coordinator will refer them to Medicaid. If client is denied by Medicaid, the program would request supporting documentation of that denial, as well.
