COVID-19 IMMUNIZATION CLINICS AND VACCINE INFORMATION

SERVICE DESCRIPTION:Provides information on how to register to receive the COVID-19 vaccine as it becomes available to the public. Inoculations are provided at immunization clinics to help prevent individuals from contracting and further spreading the COVID-19 virus. The two vaccines approved by the U.S. Food and Drug Administration require two doses, either 21 or 28 days apart.

As of December 14 2020, Ohio has started distributing COVID-19 vaccines statewide to those who choose to be vaccinated. Ohio will follow a phased approach to vaccine administration. As of March 29, 2021, all Ohioans over the age of 12 are eligible to recieve a vaccine. Homebound vaccines are available.

HOW SERVICE IS PROVIDED:Online

ELIGIBILITY:- Children and adults ages 12 and older
- Individuals under the age of 18 are only eligible to receive the Pfizer vaccine at this time
- A parent of guardian must accompany individuals under 18 years of age

SERVICE HOURS:Mon-Sun 24 hours

FEES:No fees

DOCUMENTS REQUIRED:- Photo I.D.
- Health insurance card
- Other documents may be requested
