COVID-19 VACCINE INFORMATION AND IMMUNIZATION CLINICS

SERVICE DESCRIPTION:Provides updated information on the COVID-19 vaccine and where to obtain the COVID-19 vaccine.

HOW SERVICE IS PROVIDED:- Online: https://sites.google.com/view/wchdohio-coronavirus/home/vaccines/get-vaccinated?authuser=0

ELIGIBILITY:Anyone 6 months of age and older.

SERVICE HOURS:Website registration available Mon-Sun 24 hours

FEES:- There will be no out-of-pocket costs to individuals for the vaccine
- Healthcare providers may bill insurance for administrative costs

DOCUMENTS REQUIRED:- Government issued photo identification
- Insurance card, if applicable
- Other documents may be requested

HOW TO ACCESS:Registration not required - all clients accept walk in appointments
