SERVICE DESCRIPTION:Provides services for people to obtain birth for any person born in Ohio and death certificates for anyone who passed away in Fulton County. Due to changes in State law, we can no longer provide uncertified copies of birth or death certificates.

HOW SERVICE IS PROVIDED:On site, by mail and online

ELIGIBILITY:No restrictions

SERVICE HOURS:Mon-Fri 8:30am-4:30pm

FEES:The fee for certified copies of birth and death certificates is $23 per copy

DOCUMENTS REQUIRED:Cash, money orders, or check payment with valid Picture ID
