VITAL STATISTICS

SERVICE DESCRIPTION:Operates a statewide system for the registration of births, deaths, and other vital events. Defiance County General Health District provides services for people to obtain birth and death certificates.

HOW SERVICE IS PROVIDED:On site, by mail and online

ELIGIBILITY:Provides certified copies of birth certificates available for any person born in Ohio and certified death certificates available for individuals who died in Defiance County.

SERVICE HOURS:Mon-Fri 8:30am-4:15pm

FEES:The fee for certified copies of birth and death certificates is $25 per copy. Does not accept out-of-state checks and there is an additional $2 charge for credit/debit cards.

DOCUMENTS REQUIRED:Cash, money orders or check payment with a valid Picture ID
