SERVICE DESCRIPTION:Provides a one hour safe-sleep class. At the end of the class, parents/caregivers are provided with a Graco Pack ‘n Play portable crib. Also offers a complete ‘Safe Sleep Survival Kit’, which contains a Halo Sleep Sack (wearable blanket with Back to Sleep message), a Pack n Play crib sheet, and a pacifier.

HOW SERVICE IS PROVIDED:After a virtual class is completed, upon proff of completion, an appointment can be schduled for pick up

ELIGIBILITY:Women 37 weeks pregnant or with an infant under 5 months of age, who are currently enrolled in, and referred by, one of the following programs: Baby University Compassion Health Toledo Chrysalis East Toledo Family Center Ebeid Center Family and Child Abuse Prevention Center Family House Healthy Start Hospital Council of NW Ohio (Pathways/Carenet) Help Me Grow La Posada Lucas County Children’s Services Mercy Healthy Connections Toledo Healthy Tomorrows University Church YWCA
