60-PLUS SENIOR HEALTH ASSESSMENT CLINIC

SERVICE DESCRIPTION:Provides free health assessments conducted by a Public Health Nurse. Services include blood pressure check, cholesterol screening, blood test for anemia, medication counseling, blood glucose testing, weight monitoring, diet information, health education and referral to physicians, if necessary.

HOW SERVICE IS PROVIDED:At several sites in Ottawa County.

ELIGIBILITY:Residents of Ottawa County, age 60 and older

SERVICE HOURS:M-F: 9:00am-3:00pm.

DOCUMENTS REQUIRED:Bring medications or a list of medications

FEES:None
