SERVICE DESCRIPTION:Provides vaccines for children whose health care provider does not offer them.

HOW SERVICE IS PROVIDED:On site

ELIGIBILITY:Children age 0 to age 18 whose provider does not offer vaccines

SERVICE HOURS:Hours vary

FEES:Most insurance plans including Medicaid. If cost of vaccines is an issue, a sliding fee scale based on income and family size.
