SERVICE DESCRIPTION:Provides immunizations for children.
HOW SERVICE IS PROVIDED:At various sites in the community.
ELIGIBILITY:Birth through age 18.
SERVICE HOURS:Varies on location
FEES:$21.25 administration fee per shot for those whose insurance does not cover any vaccines or for those without any insurance. Does not turn away for inability to pay
DOCUMENTS REQUIRED:For each visit, parents must bring a paper copy of child’s immunization record, ID and insurance card. If parent is not able to bring child a note of consent must be sent with the adult.
