SERVICE DESCRIPTION:Healthy Start is a free federally-funded community care coordination and home visiting program serving pregnant individuals, families with children under 18 months, and eligible men/co-parents in Lucas County. Services include care coordination, home visiting, health education, pregnancy and parenting support, reproductive life planning, referrals to community resources, assistance accessing health care and insurance, breastfeeding support, and connections to baby supplies and other needed resources. Healthy Start also offers co-parenting workshops, trauma-informed mental health support groups, and one-on-one counseling for women. The goal is to reduce preterm birth, low birth weight, infant mortality, and improve healthy birth and child development outcomes.
HOW SERVICE IS PROVIDED:By phone, on site, where client is located, or throughout the community
ELIGIBILITY:- Women who are pregnant, interconception, and preconception (ages 13-44)
- Children newborn-18 months
- Men with enrolled mother or child
- No income or insurance requirements
SERVICE HOURS:Mon-Fri 8am-4pm
FEES:No fees
DOCUMENTS REQUIRED:A signed consent form
