SERVICE DESCRIPTION:Provides financial aid for residents of Oklahoma and Canadian counties who have lost jobs or hours of work due to COVID-19. There is a back up so this is not a quick process. Must apply online.
SERVICE HOURS:Application 24 hr. Office Mon-Fri 8am-4:30pm

ELIGIBILITY:Picture identification for applicant and spouse, social security number, proof of residency, proof of Income required for all adults in household, For clients that have lost their job due to COVID-19, please provide at least one of the following in addition to last paystub: (example notice of furlough or layoff, unemployment determination, bank statement evidencing recent income loss, etc).
" For clients that have lost hours due to COVID-19, please provide check stub before and after loss of hours.
9. If applying for a Gas Voucher, must have valid driver's license and valid vehicle insurance.
10. Rent can be past due, provide late or eviction notice. Must be in applicants or spouse name. Provide copy of lease agreement.
11. Mortgage can be past due, provide the monthly mortgage statement showing past due amount. Those under a Forbearance agreement are not eligible.
12. If applying for Utility Assistance, utility bill must be in applicant or spouse name.
13. The utility may be past due or cut-off status, we will not pay if the utility is currently off.
14. No deposits or reconnect will be paid.

SITE DESCRIPTION:Provides over 43 service programs including but not limited to, home buyer and home owner programs, Head Start, weatherization of homes, emergency home repair, homeless service, outpatient counseling for alcohol and drug use, prescription assistance. Does not operate with FEMA.
