SERVICE DESCRIPTION:Provides supportive services to very low-income Veterans and their families residing in or transitioning to permanent housing. Provides financial assistance and case management services for households at-risk of becoming homeless and re-housing assistance for homeless veterans and their families.
SERVICE HOURS:Mon-Sun 24 hours for prescreening
INTAKE PROCEDURE:Must call 2-1-1 to complete pre-screen survey with 2-1-1 Call Specialist for referral to program. All SSVF referrals must first come through 2-1-1 Heartline pre-screen survey.

ELIGIBILITY:Veterans and their families who meet residency, income, housing and discharge criteria on 2-1-1 pre-screen.
