SERVICE DESCRIPTION:Provides screening, assessment and referral process to families or individuals who are literally homeless, and connect them with case management for housing solutions.

SERVICE HOURS:Mon-Sun 24 hours for prescreening assistance

INTAKE PROCEDURE:Must complete ESG Prescreen Form, and consent a Follow Up.

FEES:None

ELIGIBILITY:Must reside in Oklahoma City and be living in the streets, homeless shelter or car

DOCUMENTS REQUIRED:None
