SERVICE DESCRIPTION:Providing day care, educational, full medical and dental services and nutrition and social services to children aged 3-5 with parents included.

SERVICE HOURS:Mon-Fri 8am-5pm

INTAKE PROCEDURE:Call for information or go to the website, http://www.caaofokc.org for a list of all the Head Start programs currently available

FEES:Call for fee schedule.

ELIGIBILITY:Children aged 3-5 years.

DOCUMENTS REQUIRED:Need to bring proof of income (check stubs for past 3 months), proof of residency no older than 30 days, picture ID, Social Security cards for all persons living in the household.
