SERVICE DESCRIPTION:There are a number of different places to get the vaccine. All are required to stay in the current Phase of who is eligible except Tribal health centers. Here are the links to the different vaccine portals:

SERVICE HOURS:Varies by location

INTAKE PROCEDURE:Use the online portals to schedule an appointment. Each portal is independent so signing up in one will not give you information about the others. You will need to check all the portals to find available appointments in your area.

ELIGIBILITY:Anyone age 12 or older can have the vaccine. Those under age 18 need to have a parent or guardian with them.
