SERVICE DESCRIPTION:Provides outpatient counseling services, including family or individual therapy, for clients ages 0-21 who are Medicaid eligible, or clients that currently have private insurance. Clients can be seen at the office or school based.
SERVICE HOURS:By appointment.
INTAKE PROCEDURE:Intakes scheduled from referrals.
FEES:None
ELIGIBILITY:Must either be Medicaid eligible, OJA referred, have private insurance or attend public schools.
DOCUMENTS REQUIRED:Please call ahead for document verification.
