SERVICE DESCRIPTION:Enables low-income families to buy nutritious food with Electronic Benefits Transfer (EBT) cards. SNAP recipients spend their benefits to buy eligible food in authorized retail food stores/Farmers' Markets. Participants are also eligible for OK SNAP Works that is a vocational program that also offers child care, transportation assistance, work cloths.
SERVICE HOURS:Mon-Fri 8am-5pm
INTAKE PROCEDURE:Apply online at http://www.okdhslive.org/ or submit a completed form 08MP001 to your local DHS office. The form can be found online or at the DHS office.

ELIGIBILITY:Must be: US Citizen or Legal Alien, unemployed adults ages 18-50, meet income standards and have a social security care for each member of the household.
DOCUMENTS REQUIRED:Photo ID, social security numbers for each member of the household, verification of all income received by your household for at least the last 30 days (includes pay stubs, self employment recors or tax return, child support payment receipts, award letter from social security income, proof of unemployment benefits, money received from family or friends and any other income received). If you have any of the following it may increase the amount of SNAP benefits received: shelter and utility costs, child care expenses, child support payments, medical expenses (if your household has an elderly or disabled member).
