STATE CERTIFIED ASSESSMENT
SERVICE DESCRIPTION:Provides an assessment for addiction treatment using the ASAM Service Level for clients seeking state funded residential addiction treatment. Provider will be able to tell the client of all the available beds in the state for residential treatment, or referral for detox or outpatient services.
SERVICE HOURS:Mon-Fri, 8am-5pm
INTAKE PROCEDURE:Call to schedule an appointment
FEES:Sliding scale based on income or insurance coverage.
ELIGIBILITY:Accepts individuals without insurance or those with Medicaid/Soonercare
DOCUMENTS REQUIRED:Must provide photo ID/driver's license

SITE DESCRIPTION:Provides outpatient behavioral health services to children, families, and adults. Works with OKDHS to provide home-based child abuse prevention services.
