SERVICE HOURS:Mon-Thu 8:30am-5pm Fri 8:30am-3pm
INTAKE PROCEDURE:Done electronically before the first appointment.
FEES:Private pay, Medicaid, and Memberships for uninsured.
ELIGIBILITY:Adults, Children, and Adolescents.
DOCUMENTS REQUIRED:Must provide Social Security Card, Insurance Card, Proof of ID
