INTAKE PROCEDURE:Done in office or for crisis may be done in the field.



FEES:Private pay, Medicaid, Private insurance, and memberships for uninsured.

ELIGIBILITY:No restrictions

DOCUMENTS REQUIRED:Must provide Social Security Card, Insurance Card, Proof of ID
