ELIGIBILITY:Must be enrolled as a tribal member of any federally recognized tribe and reside in service area. Priority is given to older adults, individuals with a disability, and households with small children.

DOCUMENTS REQUIRED:Must provide tribal membership card, proof of residence, and proof of income.
