Provides assistance in the enrollment of members who are descendants of Seminole citizens whose name appears on the final rolls of the Seminole Nation of Oklahoma.

ELIGIBILITY:Must be able to prove blood linage to Seminole Nation Tribe.

DOCUMENTS REQUIRED:Must provide a Certificate of Degree of Indian Blood (CDIB) card, an 8x10" long form showing direct lineage to an original enrollee of the Final Seminole Dawes Roll by blood, an original state-certified birth certificate, a copy of Social Security Card(s), adoption/custody records (if applicable), and a notarized application form. If enrolling by mail, adult applicants will need to send a clear individual photo (size 3x5 or 4x6) along with their signature on a plain white sheet of paper.
