SERVICE DESCRIPTION:Provides subsidized child care benefits for children while their parents or guardians are at work, in training, trying to find employment or receiving an education. Subsidized child care benefits may also be provided as part of a protective service plan to prevent abuse, neglect or exploitation. The subsidy is paid directly to the child care provider on the family's behalf.
SERVICE HOURS:Mon-Fri 8am-5pm
INTAKE PROCEDURE:Application available online.
FEES:Co-payments based on income and the number of persons in family.
ELIGIBILITY:Must reside in service area, meet income guidelines, and have a verified need. Children must be 13 years of age or younger; children with disabilities may qualify up to the day before the child's 19th birthday.
DOCUMENTS REQUIRED:Must provide identity verification for the person making the application, proof of income, names of all household members, work and/or school schedule for all adults in the household showing the days and hours of work and/or school (if schedule varies each week, the schedule must reflect this), and the name of child care provider the applicant wishes to use.
