REHABILITATION / HABILITATION SERVICES

Provides community services for individuals with developmental disabilities and their families including family support assistance, group homes, sheltered workshops, community intergrated employment, guardianship assistance, assisted living, and a voucher respite repayment program.

ELIGIBILITY:Must be 3 years of age or older, have a primary diagnosis of intellectual disabilities, and meet Medicaid Long Term Care criteria.

DOCUMENTS REQUIRED:Must provide birth certificate, school records that document the intellectual disability presented prior to 18 years of age, financial records, Social Security and Medicaid benefit information.
