SERVICE DESCRIPTION:Provides 24 hours a day services for people with severe and persistent mental illness. Services are provided to clients at their homes, work or in a community setting with goal of reducing hospitalization, homeless and incarceration. PACT Teams include: psychiatrists, nurses, licensed therapists, certified case managers and recovery support specialists.

SERVICE HOURS:Mon, Wed 8am-5pm; Tue, Thu 8am-7pm; Fri 8am-3pm

INTAKE PROCEDURE:Referral by school, doctor, Department of Human services case worker or guardian

FEES:Accepts Medicaid/Soonercare, sliding scale

ELIGIBILITY:Individuals with serious mental illness which severely impairs their functioning in community living, must be 18 years of age or older.

DOCUMENTS REQUIRED:Call ahead for required documents.

SITE DESCRIPTION:Provides outpatient behavioral health services to children, families, and adults. Works with OKDHS to provide home-based child abuse prevention services.

SITE HOURS:Mon-Thu 8:00am-5:00pm; Fri 8:00am-3:00pm
