ADULT COMMUNITY BASED SERVICES

Provides 24 hours a day services for people with challenges engaging in traditional office-based services. Services are provided to clients at their homes or in a community setting with goal of reducing hospitalization, homeless and incarceration. PACT Teams include psychiatrists, nurses, therapists, case managers, peer support staff and wellness coaches.

ELIGIBILITY:Individuals with serious mental illness which severely impairs their functioning in community living, must be 18 years of age or older.

SITE DESCRIPTION:Provides outpatient behavioral health services to children, families, and adults. Works with OKDHS to provide home-based child abuse prevention services.
