SERVICE DESCRIPTION:Helps identify and refer seniors who are at-risk. Once referred, a response team mobilizes to respond to the senior, assesses the needs and links the individual to community resources; must be 60 years or older and live in Oklahoma County only.

SERVICE HOURS:Mon-Fri 8:00am-5:00pm; Help Line 24-hours

INTAKE PROCEDURE:Telephone.

FEES:No charge.

ELIGIBILITY:60 years and older living in Oklahoma County.
