Provides assistance for people to remain in their own home for as long as possible by matching an individual's needs with community resources. Based on assessment, the case manager works with the individual and the family to identify areas where assistance is needed and then develops and coordinates an individualized plan that provides information and referral, linkage to resources and services, monitoring and follow-up to fill the gaps that threaten independence and safety. Also provides assistance with applications for other programs, e.g., the ADvantage program.
